As someone has yet to start a thread here, I thought I'd start the ball rolling by asking for your opinion on the following guestbook:

For starters, you should install a demo onto your site again ;) , so people could see what's it about, before having to install it themselfs.

The code seems to be in order, but I don't like the lay-out and formatting too much. Can this be adjusted easily?

The whole thing is 100% template based... All the templates are standard HTML, which are parsed upon request and populated with data.

Example:
(templates/entries.html)
<HTML>
<head>
<title>%%TITLE%%: View Entries</title>

Would give you:
(guestwho.cgi?action=view)
<HTML>
<head>
<title>Your Guestbook: View Entries</title>

(there's dozens of configurable options, read the GuestWho Manual (http://gatsby.tafe.tas.edu.au/guestwho/docs/manual.doc)

For a simple guestbook i find a Tagboard or a shoutbox is a better way :)
