Hi, I'm looking for the following written in php for mysql:

Setup an authentication page in which users will log in using a unique code stored in a mysql table [we'll have a pre-existing database of users that we'll mail this unique code out to].

Setup a page where new users can post information (name, address, city, zip code, etc...) to the database.

Setup a survey page where answers will be posted to a table in the database - I'll add in the questions later, I just need a few examples for "multiple choice" and "open ended" questions.

Setup a query page for the owner/admin to check results.

All pretty simple stuff, and I'd like to be able to modify the code myself - so good writing skills/notations are a plus.

Timeline is one week from today [9/10-9/17]
